Official Rules
NO PURCHASE NECESSARY. VOID WHERE PROHIBITED. A PURCHASE WILL NOT IMPROVE YOUR CHANCES OF WINNING. SWEEPSTAKES BEGINS FEBRUARY 22, 2010 AND ENDS APRIL 30, 2010.
How to Enter:
Mail-in Entry: To enter the Particulate Matters Sweepstakes (the "Sweepstakes"), print your full name, complete home mailing address (no Post Office Boxes) including zip code, county of residence, daytime phone number and birth year on a 3x5 post card and mail it to: Particulate Matters, c/o Illinois EPA, 1021 North Grand Avenue East, #35, Springfield, IL 62702. All mail entries must be hand-printed and legible. Mailed entries must be postmarked no earlier than February 22, 2010 and no later than April 30, 2010 and received no later than the close of business on May 7, 2010 to be eligible for the Sweepstakes. PLEASE NOTE: All eligible entries, including those received via U.S. mail, are added to an electronic database. Mail-in entries will be entered by the date they are postmarked. ONLY ONE ENTRY IS PERMITTED PER PERSON DURING THE SWEEPSTAKES REGARDLESS OF METHOD OF ENTRY. Winners are randomly selected from this electronic database. Entering by mail will in no way increase the chances of winning.
Online Entry: To enter online, log onto www.particulatematters.org, or find a link to the sweepstakes on www.nbc5.com, www.telemundochicago.com or www.cleantheair.org. Click on the sweepstakes link and complete the online particulate matter quiz and online entry form. Only one email address per individual may be used for the Sweepstakes; individuals using multiple email addresses risk disqualification of all entries. ONLY ONE ENTRY IS PERMITTED PER PERSON DURING THE SWEEPSTAKES REGARDLESS OF METHOD OF ENTRY.
The online entry form will ask individuals to answer air quality awareness questions prior to completing the entry form for the Sweepstakes. These responses will be used by the Administrator to gauge public awareness on air quality. The online form will also ask entrants to indicate where they learned about the Sweepstakes and will allow them to mark checkboxes to receive additional information. To unsubscribe from the email list, send an email to the following address: EPA.Partners@illinois.gov. All online entries must be received before 11:59 pm on April 30, 2010 to be eligible for the Sweepstakes.
MULTIPLE ENTRIES WILL NOT BE ACCEPTED. Sweepstakes entries are checked regularly to ensure that entrants are not violating this rule. In the event an entrant is found to have submitted more than one entry during the Sweepstakes, the Administrator will disqualify any and all entries submitted by that entrant for the entire 2010 Particulate Matters Sweepstakes. The Administrator reserves the right to disqualify entries for the violation mentioned above without any obligation to notify the entrants of their disqualification.
Incomplete entries will be disqualified.
Eligibility:
The Sweepstakes is open to persons who are eighteen (18) years of age or older at time of entry. Entrants must be legal U.S. residents of one of the following Illinois counties: Cook, DuPage, Kane, Lake, McHenry, Will, Grundy or Kendall. Employees of Administrator, Agency and Sponsors—American Lung Association of Illinois (the "Administrator"), Illinois Environmental Protection Agency (the "Agency"), General Electric Company, NBC Universal, Inc., WMAQ Television/NBC5, WSNS Television/Telemundo44, WBBM-AM, WUSN-FM, WXRT-FM (the "Sponsors") and any of their respective affiliates, volunteers, promotion or advertising agencies or any other company involved with the design, production, execution or distribution of the Sweepstakes and immediate families of household members of each of the foregoing are ineligible to enter. "Household member" shall mean people who share the same residence at least three (3) months a year. Void where prohibited. All federal, state, and local laws and regulations apply.
Selection of Winners:
Prize Winners: Three (3) Winners will be randomly selected from all eligible entries received to date. A representative of the Agency will contact the Winner(s) based on information provided on the submitted entry form(s); the Prize(s) will be sent via United Parcel Service or United States Postal Service to the address provided by the Winner(s). If the Agency is unable to contact Winner(s) from the information provided after a reasonable effort has been made, it reserves the right to select another Winner(s) at its sole discretion. All decisions of the Sponsors, Administrator and Agency are final and binding on all matters relating to the drawing and the Sweepstakes generally.
Only one (1) Prize per household for the duration of the Sweepstakes.
Odds of winning depend on the number of entries received.
Prizes:
Prize: Three (3) Winners will be randomly selected from all eligible entries received to date. Each Winner will be awarded an ENERGY STAR appliance or product valued up to $600. Winners will pick the ENERGY STAR appliance or product of their choice (electronics and computers excluded) from a list of selected prize options. This list will include items such as dishwashers, washing machines, water heaters and air conditioners.
The ENERGY STAR appliance(s) or product(s) options will be chosen by Agency and/or Sponsor in its sole discretion. Approximate Retail Value ("ARV") of each prize is $600. Prize may vary due to availability, or other circumstances. Colors and specific details of prizes shall be determined by Agency and/or Sponsor in its sole discretion. Winners are responsible for additional options. Sales tax and all other expenses on the receipt and use of prizes are the sole responsibility of the Winners. Winners are not eligible for any cash rebate programs. Each prize will be delivered to Winner’s residence on a date to be determined based on availability. Winners agree that the entities and individuals involved with Sweepstakes have neither made nor are responsible for any warranty, representation, or guarantee, expressed or implied, in fact or in law, relative to the prizes, including, but not limited to their quality, condition, or fitness.
General:
By entering, entrants agree to be bound by and acknowledge compliance with these Official Rules including, without limitation, eligibility requirements and the decisions of the Sponsor, Administrator and Agency. In the event any prize is unavailable for any reason, Sponsor, Administrator or Agency reserve the right to substitute a comparable prize of equal or greater value. Prizes may not be assigned, substituted, transferred, changed or redeemed for cash. Failure to comply with these Official Rules will result in disqualification. Taxes on prizes, if any, are the sole responsibility of Winners. Winners must sign and return Affidavit of Eligibility and Liability/Publicity Release within five (5) business days after receipt. Failure of any Winner to return the executed Affidavit and Release form within the specified time period, non-compliance with these Official Rules or return of the Sweepstakes notification as undeliverable will result in that Winner’s disqualification and forfeiture of the prize, and an alternate Winner(s) will be selected from remaining eligible entries for the applicable drawing. By accepting the prize, the Winners each (i)release the American Lung Association of Illinois (the "Administrator"), Illinois Environmental Protection Agency (the "Agency") and General Electric Company, NBC Universal, Inc., WMAQ Television/NBC5, WSNS Television/Telemundo44, WBBM-AM, WUSN-FM, WXRT-FM (the "Sponsors") and their respective officers, directors, stockholders, affiliates, agents, employees, advertising agencies, and all others associated with the development and execution of this Sweepstakes from and against any and all liability with respect to or in any way arising from this Sweepstakes and/or acceptance or use of any prizes, including, without limitation, liability for personal injury, damages or loss, and (ii)consent to use of their names, voices, and likenesses for promotional advertising, publicity or trade purposes in any and all media worldwide in perpetuity by the Administrator, Agency and Sponsor without further compensation, unless otherwise prohibited by law. Any person who supplies false information, obtains entries by fraudulent means, or is otherwise determined to be in violation of these rules in an attempt to obtain any prize will forfeit any prize won and will be prosecuted to the fullest extent of the law. All applicable entities, the Administrator, the Agency and the Sponsors are not responsible for any typographical error in the printing of these Official Rules, administration of the Sweepstakes or in the announcement of the Sweepstakes prizes.
